Karl Smith
Leading with experience, caring with heart.
Dr. Smith has practiced for more than 20 years, served as President of the Pierce County Dental Society, and continues to study with the Spear Study Club so his patients benefit from dentistry that keeps moving forward.
Ask him what he's most proud of, though, and he'll tell you about the patients who've trusted this practice long enough to bring in their families and friends.
Here for you in every chapter
Dentistry NW has been a fixture in Tacoma for over 20 years, and some of our earliest patients now bring their own kids through our doors. Led by your Tacoma dentist, Dr. Karl Smith, we combine decades of clinical expertise with a philosophy that puts people first.
Whether you're seeking a long-term dental home or life-changing reconstructive work like implants and All-on-X, we provide high-caliber dentistry in a space designed for your comfort.
Total dental health,
tailored to you
Cleanings, exams, fillings, extractions, root canals, and gum disease treatment, without unnecessary add-ons. We tell you what needs attention now and what can wait, showing you the x-rays either way.
Veneers, SureSmile Clear Aligners, and teeth whitening for your smile. Botox for TMJ disorder and cosmetic concerns. Dental work and aesthetic treatment in the same office, with the same dentist who already knows your history.
Crowns, bridges, dental implants, All-on-X, full-mouth rehabilitation, and bone grafting, including socket preservation. Dr. Smith places and restores implants in-office and coordinates directly with Tacoma-area specialists on complex cases.

Are you accepting new patients?
Yes. Dentistry NW is currently welcoming new patients in Tacoma, Fircrest, and University Place. Call (253) 367-3972 or request a time online; we'll respond the same day with options that fit your schedule, including early morning appointments from 7am.
What makes Dentistry NW different from other Tacoma dental offices?
Three things that are harder to find than they should be: a dentist with 20+ years of experience who still takes continuing education seriously, a practice that has operated continuously in Tacoma since 2002, and a no-upsell policy that's been part of how this office runs since the beginning.
What's the difference between fee-for-service and using insurance at your Tacoma dental practice?
With insurance, your plan has partial authority over what gets authorized, when, and in what sequence. Fee-for-service means the clinical conversation stays between you and Dr. Smith about what your teeth need, not what the plan will cover.
What happens at the first visit?
A comprehensive exam, full-mouth x-rays, and a cleaning, plus about twenty minutes of conversation before any of it starts. We ask what's been bothering you, what your previous dental visits were like so we can tailor your experience to you, and what you're hoping to get out of the relationship. Many dental offices skip that part. We don't, because the clinical picture is clearer when we understand the context.
I haven't been to a dentist in years. Is that going to be a problem?
No. We see this regularly; the ADA estimates roughly 100 million Americans have no regular dental provider. Gaps in care are common, and we're not going to make you feel bad about yours. We'll assess where things stand, explain what we find, and build a plan from there. The only thing that makes a long gap worse is extending it.
Do you treat dental anxiety?
Yes. Nitrous oxide sedation is available for patients who need it, and appointments are paced around your comfort rather than our schedule. Studies estimate that 36% of the U.S. population experiences dental fear significant enough to delay care, including patients who've had painful experiences at other offices. If that's you, tell us before you come in. We'll set things up accordingly.
Do you handle complex cases like implants and full-mouth rehabilitation, or will I be referred out?
Both, depending on what's needed. Dr. Smith places and restores implants in-office and performs bone grafting, including socket preservation. For cases requiring an oral surgeon or periodontist, he coordinates directly with a network of Tacoma-area specialists he's worked with for years, so you're not starting over with someone who doesn't know your history.
How often should I come in?
Every six months for most patients. If there's active gum disease, a cracked tooth we're monitoring, or treatment in progress, we'll recommend a cadence that fits your situation specifically. The American Dental Association supports individualized recall schedules over blanket six-month rules, and so do we.
Why has Dentistry NW been in Tacoma for over 20 years?
Because the patients kept coming back and started bringing their kids, and referring friends and family. Tacoma dental practices that rely on upsells, rushed appointments, or insurance-driven treatment plans don't retain multi-generational families. The ones that do earn it through straightforward care, honest communication, and showing up the same way year after year. That's the model here, and it's worked since 2002.
